Job Description
  • Full-time Transitional Care Navigator
  • Schedule: Monday thru Friday, Full Time
  • Excellent communication, negotiation and public relation skills.
  • Ability to market effectively with customers, referral sources, and the community.
  • Responding to customer and community needs regarding education as it relates to the hospice philosophy of care and delivery of services.
  • Developing and increasing the access to Paradigm Hospice & Palliative Services within the service area.
  • Assist, facilitate and coordinate between our referral sources, business development associates, our intake team and our patients and their families, the referral and admission process to promote a continuum of care model.
  • Facilitate the consent and referral to admit process.
  • Serve as a resource to review and provide available patient information related to a referral.
  • Ensure coordination and communication with referral sources and hospice agency leadership once consents are completed.
  • Serve as liaison between assigned hospital(s), skilled nursing facilities, community and hospice programs, with coordination of referrals/admission process and services.
  • Promptly respond to customer, both internal and external, and referral source requests and concerns
  • Compliance with accepted professional standards and practice.
  • Self-directed with the ability to work with little supervision.
  • Flexible and cooperative in fulfilling all obligations.
  • Ability to be creative and "think outside the box" and have fun doing so.
  • Great organization skills.
  • Develop relationships in the community on behalf of the company.
