Description

American Traveler is hiring a Home Health RN Float for a 13-week assignment requiring 1 year of certified home health care experience and 1 year of case management experience.

Details
  • Home health setting covering the North Region including Back Bay, Beacon Hill, Boston, Cambridge, Charlestown, Chinatown, Fenway, Financial District, Kenmore, North End, Prudential, South Boston, South End, and West End
  • Float position requiring travel to patient cases as assigned throughout the coverage area
  • Day shift schedule from 08:00-16:30
  • Weekend rotation required once every 6 weeks
  • 13-week assignment length
  • Uses Epic for charting
  • Uses UKG/Kronos for timekeeping
Requirements
  • Active MA RN license required (no conditional or pending licenses accepted)
  • Both NURSYS and BON verification required for nursing license
  • Current BLS certification (AHA only)
  • Minimum 1 year of experience specifically within a certified home health agency required (hospice, private care, or standalone case management experience does not qualify)
  • Minimum 1 year of case management experience required
  • Ability to travel to patient cases as assigned
  • Flu vaccine required with no religious or medical exemptions allowed
Additional Information
  • Assess patient conditions, implement individualized care plans, and provide ongoing education and support to patients and families
  • Perform admission processes and case management responsibilities in accordance with organizational policies and standards of practice
  • Complete thorough and timely electronic documentation and interdisciplinary communication
  • Deliver patient care within organizational philosophy, policy, and standards of community health practice
  • Start date is contingent on credentialing clearance and may be rescheduled if not cleared by the posted start date
  • Local candidates residing within 60 miles of Boston are accepted at a reduced rate
