Description

American Traveler is seeking a Weekend Field RN for a PACE (Program of All-inclusive Care for the Elderly) setting in Michigan, providing geriatric home health and field nursing care on Saturdays and Sundays.

Details
  • Weekend-only schedule: Saturday and Sunday, 7:00am โ€“ 11:00pm
  • PACE (Program of All-inclusive Care for the Elderly) setting serving a frail and elderly participant population
  • Field-based role involving regular home visits for wound care, IV hydration, lab draws, and nursing assessments
  • Case mix includes routine, acute, and non-acute visits across emergent, urgent, non-urgent, and home care status levels
  • Coordination required with PCPs, DME suppliers, ALFs, SNFs, pharmacies, hospitals, paramedics, and other contracted partners
  • EMR documentation required for all field visits, calls, contacts, and incident reports
  • 13-week contract
Requirements
  • Active Michigan (MI) RN license currently in hand
  • Current CPR certification (AHA) required, or willingness to obtain upon hire
  • Minimum of 1 year of experience working with frail, elderly, or long-term care populations
  • Associate's degree in Nursing from an accredited school required; BSN preferred
  • Valid Michigan driver's license, reliable personal transportation, and current auto insurance required
  • Must be 18 years of age or older
  • Full vaccination required or willingness to vaccinate against: Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B
Additional Information
  • Conduct in-person participant assessments to determine appropriate levels of care in alignment with individualized care plans
  • Perform full-body assessments using critical thinking skills during home and field visits
  • Respond promptly to calls from PCPs, triage staff, and support staff, documenting all contacts and follow-ups
  • Initiate emergency treatments and document new orders, including fall reports, incident reports, medication errors, and grievances as needed
  • Provide education and support to participants, family members, DPOAs, and facility staff using a participant-centered care model
  • Actively participate in orientation, in-service programs, staff meetings, and quality improvement initiatives
  • Home care experience (1+ year) and wound care experience (1+ year) are preferred
  • Must be adaptable to changing workloads and team priorities, and maintain open communication with leadership
