Requirements
  • Active Registered Nurse (RN) license in the state of Louisiana (or compact license eligibility).
  • Minimum of 1 year of full-time Home Health RN or Home Health RN Case Manager experience in the past 2 years; home health experience strongly preferred.
  • Proficiency with HCHB (Homecare Homebase) EMR and experience completing OASIS documentation.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and automobile liability insurance.
  • Relevant education and credentials must be provided with your application for consideration.
About This Role
In this assignment, you will deliver compassionate, patient-centered nursing care to individuals in their homes. You'll conduct comprehensive assessments, create and implement personalized care plans, supervise LPNs and HHAs, and support patients and families as they navigate health challenges.
Schedule:
Monday - Friday, 8:00 AM - 5:00 PM (on-call rotation may be required; hours may vary)
