What You'll Do

As a Home Health RN Case Manager, you'll combine hands‑on nursing care with clinical case management, helping patients safely recover and manage their health in the comfort of their own homes.

Your responsibilities will include:

  • Manage an assigned caseload within the Northwest Phoenix Valley
  • Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments
  • Perform comprehensive nursing assessments and develop individualized plans of care
  • Provide skilled nursing care based on each patient's clinical needs
  • Perform wound care and monitor healing and treatment effectiveness
  • Provide medication management, reconciliation, and patient education
  • Monitor changes in patient condition and communicate promptly with physicians and the interdisciplinary care team
  • Educate patients and caregivers on disease management, medications, safety, and prevention of complications
  • Coordinate care with Physical Therapists, Occupational Therapists, Speech Therapists, Medical Social Workers, Home Health Aides, physicians, and other healthcare professionals
  • Complete timely and accurate documentation in accordance with Medicare, regulatory, and agency requirements
  • Help prevent avoidable hospitalizations and emergency department visits
  • Support patients in achieving greater independence, safety, and quality of life at home

QualificationsRequired
  • Current Arizona Registered Nurse (RN) License or Compact RN License valid in Arizona
  • Graduate of an accredited nursing program
  • Current CPR Certification
  • Valid driver's license
  • Reliable transportation and current auto insurance
  • Strong clinical assessment, communication, and organizational skills
  • Ability to work independently in a home-based clinical environment while collaborating with an interdisciplinary team
Preferred
  • Home health experience
  • OASIS documentation experience
  • Case management experience
  • Experience with wound care, chronic disease management, or post-acute care
