Minimum Requirements

  • Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active West Virginia state physical therapy license required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • Home health experience required.
  • Experience with HCHB (Homecare Homebase) required.
  • Minimum education requirement: Bachelor‘s Degree.
  • Dress code scrub color: navy blue (client-specific requirement).


Preferred Qualifications

  • Additional home health clinical experience preferred.
  • Prior use of HCHB preferred beyond basic competency.


Assignment Details

  • Shift hours: 0800-1600.
  • Schedule: 8 hours per day, 5 days per week. Mon-Fri 8:00 am–4:30 pm noted by client.
  • Coverage areas include Putnam, Cabell, and Wayne counties.
  • No on-call or weekend requirement.
  • Mileage reimbursed between visits from first patient to last patient.
