Job description

Summary of Function
The Methodist Home is seeking a Targeted Case Manager (TCM) who is highly motivated to make a difference in the community and committed to the well-being of their clients. The TCM will utilize experience and knowledge to assess needs of clients and their families in order to link and coordinate the appropriate resources. They are responsible for managing an assigned caseload and responding appropriately to client needs in compliance with Medicaid standards. This includes but is not limited to: assessing client needs; developing, implementing and reviewing service plans; and working with other community resources in achieving client service needs. Caseloads may include diverse populations with a variety of clinical needs. The Targeted Case Manager is a positive team player that promotes a culture of inclusion and participation.
Education & Professional Requirements:
  • Bachelor's Degree from an accredited institution in a human services field with a minimum of 1 year of experience in a behavioral health setting post-bachelor's degree.
  • Certification in the following Department for Behavioral Health, Developmental and Intellectual Disabilities approved trainings to meet the requirements outlined in 908 KAR 2:260:
    • Twelve (12) hour Targeted Case Management Core Training
    • Six (6) hour Substance Use Disorder Targeted Case Management Training
    • Six (6) hour Severe Emotional Disability Targeted Case Management Training
    • Six (6) hour Severe Mental Illness Targeted Case Management Training

Targeted Case Manager will exhibit:
  1. Effective communication skills, including proper use of agency communication systems.
  2. Strong interpersonal and engagement skills.
  3. Professional maturity and respect for others.
  4. Strong organizational and time-management skills.
  5. Self-starter who requires minimum supervision after completion of training.
  6. Ability to follow through with documentation, communication and deadlines.
  7. Ability to support the agency's mission and philosophy and demonstrate respect for cultural diversity and team-centric harmony.

Job Specifications
  • Must be at least 21 years of age.
  • Valid driver's license, reliable transportation, and proof of current auto insurance.
  • Must submit to required background checks and pre-contract drug screening.
