Job description

Continuum of Care/Social Worker (Christian Pro-Life Organization)

The Continuum of Care Coordinator role is responsible for gathering information, identifying, and solving challenges of incoming client referrals, and providing casework type services to clients. The Continuum of Care Coordinator is also responsible for developing relationships with community resources and social service agencies to gather information for referrals and make the information available to clients to assist Human Coalition achieve its mission of existing to rescue children and serve families across America. Continuum of Care Coordinators are responsible for identifying and communicating eligibility requirements and processes of resources within the community to clients that we serve. This position is intended to bridge the gap between clients in need and the support offered by community and state programs.

Human Coalition is an innovative, energetic, compassion-driven non-profit organization with the purpose to end abortion.

Essential Functions

Caregiving

  • As referred by clinic staff, connecting with clients facing an unplanned pregnancy who are considering abortion to identify potential barriers/need
  • Assess the challenges that are influencing the client's decision-making process
  • Coach and connect clients to the necessary resources to relieve pressure to abort
  • Walk alongside clients and provide action steps as they work to eliminate barriers or challenges that may influence the decision for their pregnancy
  • Continue to meet with and coach clients to work through their challenges as needed
  • Connect clients to a volunteer mentor within the local church/program for further education
  • Protect clients' health information for confidentiality, authorized access for treatment, and data security
  • Perform other duties as assigned

Administration

  • Find, develop, and maintain network of resources within local churches, community organizations, state programs, and social service agencies to utilize for client needs
  • Document all client communication in client management system
  • Assess client stability and risk assessment scores
  • Work alongside other departments to address complete client care and scope of needs
  • Develop, track and measure effectiveness of Continuum of Care process per KPI's
  • Communicating via email, phone, text message, etc. for regular cadence of follow-up with clients as well as community partners
