Job Summary:

Responsible for coordinating the care of designated patients and families. Provides psychosocial assessment and updates that address the patient's evolving needs. Establishes a plan of care that provides maximum benefit to patients and their families. The goal of social work services is to be an advocate for patients across the continuum of care, connecting them with the necessary resources to ensure safe and effective care. Interviews patients and their families, coordinates and plans programs and activities to meet their social and emotional needs across the continuum of care. Provides crisis intervention and assists families in understanding and providing education around complexities of the medical situation and its impact on quality of life.

EXPERIENCE DESCRIPTION:

A minimum of four (4) or more years of experience in Social Work or related area is required. An understanding of standard concepts, practices and procedures within care coordination, required. Two (2) or more years of experience in an acute care setting is preferred.

EDUCATION DESCRIPTION:

Master's degree in Social Work is required.

SPECIAL SKILLS DESCRIPTION:

Interpersonal skills necessary to obtain information from patients, to provide counseling to patients/families, and interact effectively with patients, families and hospital personnel. Analytical skills necessary to assess patient's needs, develop associated discharge planning and develop solutions to problems concerning patients mental and physical wellbeing.

LICENSURE DESCRIPTION:

Current State of Wisconsin license as a social worker is required.
