External Liaison 

Hospital Liaison - Health Care Marketing Nurse

We are currently looking for a dynamic, energetic External Case Management professional to support our mission and join our forward-thinking team. The ideal candidate must be a well-spoken professional who is a highly organized self-starter capable of prioritizing many tasks effectively. 

Responsibilities:

  • Increase census through cultivating relationships with multiple referral sources including hospitals, LTACs, ALs, ILs, and the community at large 
  • Assess patients and work to build relationships with area hospital case managers, social workers and physicians
  • Attend hospital meetings, coalition meetings, and networking meetings to grow brand awareness
  • Reflect the quality of care and services provided, including on-site short-term, respite, and long-term care in our skilled nursing facility
  • Act as a self-starter, skilled communicator and positive motivator
  • Able to organize and prioritize many tasks effectively
  • Ability to utilize both local and company resources in the execution of job responsibilities
  • Must have the ability to network at all levels with strong drive, a passion for success, and a proven track record of census growth and development

Requirements of the ECM - External Case Manager:

  • Active: LPN or RN License 
  • Long Term Care Experience - 2 years
  • Marketing Experience - 2 years
