As the Clinical Care Coordinator you will work in conjunction with the treatment team in developing, coordinating, and disseminating continuing care plans with patients. Maintain up-to-date referral network.

Essential Duties Include:

  • Meets with patients to research discharge planning needs as presented by the treatment team.
  • Meets one-on-one with patients from admission until discharge at a minimum of one time per week to discuss case management.
  • May be assigned to conduct educational lecture or group activity.
  • Communicates with Business Development Department and referring provider to coordinate appropriate aftercare resources.
  • Under the guidance of the Clinical Director, reviews and researches aftercare resources.
  • Supports treatment team and patients in communication with referral and continuing care resources.
  • Documents continuing care plans, communications and meetings. Prepares and distributes copies of patient schedules weekly as requested.
  • Attends treatment team meetings as assigned.
  • Ability to converse in English with others to give, take and process information; extensive use of telephone.
  • Ability to maintain confidentiality of information, multi-task, coordinate projects and record/transcribe general and specific meeting minutes.
  • Ability to work independently and collaboratively.
  • Communicate with other staff to relay concerns and observations involving patient safety
  • Other duties as assigned

Education and Experience:

  • Bachelors Degree in Behavioral Science or related field preferred.
  • One years experience working in a behavioral health care facility preferred
