Key Responsibilities
  • Design, implement, and modify person-centered and recovery-based treatment plans tailored to individual client needs.
  • Conduct comprehensive assessments to continuously evaluate treatment needs, leveraging tools such as standardized assessments and collaborative goal-setting.
  • Collaborate with clients to develop vital coping strategies, problem-solving skills, and effective communication abilities to enhance their daily interactions.
  • Educate and advocate for clients by connecting them to essential community services that provide additional support and resources.
  • Monitor and manage symptoms through evidence-based interventions, ensuring clients remain engaged in their recovery process.
  • Facilitate skill-building activities focused on daily living skills, anger management, and developing healthy leisure activities.
  • Provide closure and aftercare planning, ensuring clients have a robust support network when transitioning out of active treatment.
  • Maintain confidentiality and adhere to ethical standards while navigating sensitive client information.
Patient Population / Client Focus

Qualifications
