Clarity Wellness Center is seeking a dedicated and compassionate Licensed Clinical Mental Health Counselor (LCSW, LMFT, LPCC) to join our telehealth team on a full-time basis. As a member of our mental health team, you will collaborate with other healthcare professionals to create effective treatment plans while maintaining regulatory compliance and robust documentation.

• Collaborate with physicians and other healthcare providers to communicate patient progress, needs, concerns, as well as any potential or actual complications during treatment.
• Maintain timely and accurate clinical documentation, including progress notes and treatment records, adhering to department policies and state regulations.
• Actively contribute to the development and implementation of personalized treatment plans, including discharge planning, grounded in detailed psychosocial assessments.
• Provide in-depth counseling and crisis intervention services, catering to emotional and behavioral issues faced by clients during therapy sessions.
• Assist clients with meticulous discharge planning and ensure seamless care coordination by liaising with regional service providers to facilitate ongoing treatment.
• Engage with clients’ families and support systems to promote holistic treatment approaches and comprehensive discharge strategies.

The ideal candidate will interact with a diverse client base seeking mental health services, including individuals from various cultural and socioeconomic backgrounds experiencing mental health challenges, including anxiety, depression, trauma, and substance abuse issues. Our focus is primarily on outpatient therapy, providing a supportive space for clients to address their unique struggles and work towards recovery.

• A Master’s degree in Social Work, Counseling, or Marriage and Family Therapy from an accredited institution is required.
• A minimum of two years of relevant experience in an outpatient setting is essential to effectively support our clientele.
• Proficiency in electronic health records (EHR) and strong computer skills, including experience in word processing, spreadsheet software, and e-mail communication, are vital for documentation and coordination.
• A demonstrated ability to work independently while remaining self-directed and highly organized is crucial to managing your caseload effectively.

While not mandatory, additional credentials or certifications in specialized therapeutic modalities or experience working in a telehealth environment could enhance your application. Familiarity with community resources for mental health support services will also be advantageous in facilitating comprehensive client care.
