• Deliver psychotherapeutic services to individuals dealing with anxiety, depression, post-traumatic stress disorder (PTSD), and other mental health challenges.
• Conduct thorough assessments of patients’ mental health status, evaluating emotional, behavioral, and social functioning to inform treatment strategies.
• Design and implement evidence-based treatment plans that are culturally sensitive and customized to meet the diverse needs of each patient.
• Collaborate with a multidisciplinary team, including psychiatrists and primary care physicians, to ensure a coordinated and cohesive approach to each patient’s care.
• Monitor and record patient progress, adjusting treatment plans based on individual feedback and developments in their mental health state.
• Educate patients and their families about mental health issues, available treatment options, and community resources that can provide additional support.
• Maintain confidentiality, ensure compliance with ethical standards, and seek informed consent from patients throughout the therapeutic process.

The LCSW will work primarily with a diverse patient population within our community health clinic, providing care to individuals facing mental health conditions across various demographics. • A Master’s degree in Social Work from an accredited institution is required.
• A current and valid Licensed Clinical Social Worker (LCSW) license in the state of Illinois is essential.

• Exceptional communication and interpersonal skills, enabling robust relationships with patients and collaboration with healthcare professionals.
• Proficient clinical skills, including the ability to conduct comprehensive assessments and develop tailored, evidence-based treatment plans.
• Familiarity with electronic health records (EHRs) and other healthcare software to efficiently manage patient data.
• A solid understanding of mental health conditions and the social factors that can impact patient care.
• Commitment to ethical practices and maintaining patient confidentiality.
