Position Overview

We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our dynamic team at Luminous Health System. This full-time remote position is based in Stamford, Connecticut, within our specialized Hospice Center. The LCSW will play an instrumental role in delivering high-quality mental health services, focusing on enhancing the overall emotional and psychological well-being of our patients.

Work Environment

The successful candidate will work remotely within a supportive and collaborative team environment. Monday to Friday, from 9:00 AM to 5:00 PM, you will utilize telehealth platforms to engage with patients across varied backgrounds and needs. Our remote infrastructure includes access to cutting-edge electronic health record (EHR) systems, enabling seamless management of patient information and treatment plans.

Key Responsibilities

• Provide comprehensive psychotherapy and mental health services tailored to the unique needs of patients exhibiting conditions such as anxiety, depression, and post-traumatic stress disorder (PTSD).
• Conduct thorough mental health assessments that evaluate emotional, behavioral, and social functioning to inform treatment planning.
• Develop and implement evidence-based treatment plans that are culturally competent and adapted to meet the diverse needs of our patient population.
• Engage in ongoing collaboration with multidisciplinary teams, including psychiatrists and primary care physicians, to ensure an integrated approach to patient care.
• Continuously monitor patient progress and adjust treatment strategies based on collaborative feedback and patient responses.
• Educate patients and their families about mental health conditions, treatment methodologies, and relevant community resources to foster a supportive recovery environment.
• Adhere to ethical practices by maintaining confidentiality, obtaining informed consent, and demonstrating professional integrity in all interactions.

Patient Population / Client Focus

The primary focus will be on patients experiencing chronic or terminal illnesses, where providing mental health support is crucial to their quality of life. This includes a compassionate approach to terminal care, bereavement support, and addressing mental health challenges arising from serious diagnoses. You will encounter diverse cases, enhancing both the breadth of your experience and the depth of patient care provided.

Qualifications

• Master’s Degree in Social Work from an accredited program.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of 2-3 years of direct experience providing psychotherapy and mental health support in healthcare settings.
• Demonstrated ability to establish rapport, build trusting relationships, and effectively communicate with patients and families.
• Proficient in developing detailed assessments and evidence-based treatment plans tailored to patient needs.
• Experience navigating electronic health records and other healthcare technologies to manage and document patient interactions.
• Strong understanding of mental health issues and treatments, along with sensitivity to the unique challenges faced by patients and their families.

Preferred Experience

• Experience in a palliative care or hospice setting is highly desirable.
• Familiarity with community resources and support systems to assist patients and their families beyond clinical support.
• Skills in leading educational workshops or support groups focused on mental health and wellness.

Compensation and Benefits
