This role involves providing therapeutic services onsite, primarily in the patient's home. As a home care physical therapist, you will work in diverse environments, navigating the intricacies of each patient's living space, and adapting your therapeutic methods accordingly. • Evaluate and assess patient conditions to create tailored treatment plans based on physician referrals.

• Educate patients and their caregivers about their condition, recommended treatment plans, and self-management strategies.

• Utilize evidence-based practices to maximize patient outcomes, consistently reviewing and adjusting approaches based on progress.
• Document patient progress accurately and maintain compliance with applicable regulations and standards.

The primary clientele include individuals undergoing rehabilitation from surgeries or injuries, children needing specialized therapy, and adults with chronic conditions that impair their mobility. We prioritize a patient-centered approach, recognizing each individual's unique needs and preferences, leading to a customized alignment of therapy goals and real-life applications.

• A valid Doctorate Degree in Physical Therapy (DPT) is required.
• Possession of a current state license in Physical Therapy.
• Graduate from an APTA or CAPTE-approved physical therapy program.
• Basic Life Support (BLS) certification from the American Heart Association.
• Strong verbal and written communication skills, allowing for effective patient education and collaboration.
• Proficient in computer systems and word processing required for patient documentation and scheduling.

While a minimum of 2 years of experience is required, candidates with additional experience in home health care settings or specialized treatment areas such as pediatrics or geriatrics will be highly regarded.
