Job description

This will be a Part-Time position with tons of flexibility in scheduling!!!
The Home Health Care RN - Case Manager / Field Nurse - Ocean County, NJ is responsible for the creation and monitoring of Client Plans of Care as well as the delegation of care and general supervision of professional and para professional field staff (e.g. Certified Home Health Aides - CHHAs) as assigned by the Director of Nursing (DON). This will be a Part-Time position where the majority of your schedule is made by YOU!
Duties & Responsibilities:
  • Reflects the values of DAALD Senior Care LLC dba Home Instead
  • Implements the policies and procedures established by DAALD Senior Care LLC, the state of New Jersey and the state Accrediting Body
  • Conducts initial non-medical patient assessments and reassessments (at a minimum every 60 days) and creates Plans of Care for delegation to CHHAs / Field Staff
  • Performs supervisory visits to ensure Plans of Care are being followed and the CHHAs / Field Staff have the required skills and knowledge to continue to perform their delegated tasks
  • Consults with client, family and Caregivers to provide a safe and efficient patient care environment and reviews progress as needed
  • Assess newly hired CHHAs level of clinical competency
  • Orients CHHAs / Field Staff when they are assigned new Clients
  • Reports and documents appropriate changes in clients condition and the general performance of CHHAs / Field Staff to the DON / Business Owner / Office Staff
  • Assigns tasks and tracks adherence to the Plan of Care in the CRM system (WellSky)
  • Ensures all paperwork is completed accurately and timely and turned in when and where appropriate
  • Completes Nurse Directed Services evaluations, paperwork and delegations (including return demonstrations) as required
  • Reviews new hire medical paperwork for completeness and adherence to company and state guidelines
  • Participates in staff meetings as needed
  • Completes other tasks as required and assigned by the DON / Business Owner / Office Staff

Qualifications include:
  • Current New Jersey Registered Nurse license in good standing with the NJ Board of Nursing required
  • Ability to travel / visit clients within Ocean County, NJ
  • 2 years experience in home care, hospice, visiting nurse or community health experience is preferred
  • Prior RN experience with in-home care assessments preferred
  • Excellent written and verbal communicator with strong interpersonal skills
  • Ability to work in a collaborative and dynamic environment
  • Must be self-motivated, organized, and able to work independently
  • Excellent computer skills
  • Holds a valid driver's license and a reliable car and willing to travel to clients' homes and the office as needed

Other Conditions and Essential Functions:
Work is in a variety of home environments. Travel by car throughout the service area is
necessary to provide essential tasks. Tasks may involve exposure to blood, body fluids
or tissue (OSHA Category I). This position requires lifting, bending, standing, walking
up and down stairs, seeing, hearing, speaking and writing.
