Our agency will employ direct care staff that are certified nursingassistants at our PHCP agency. Our direct care staff will take care ofindividuals' ADLs, household management, community-based shopping andappointments; working closely with family members and others to supportClients' progress towards better health and well-being. All certified nursingassistant staff report directly to our Administrator, and are supervised andmonitored by our qualified RN.
Duties and Responsibilities:
1. Assistresidents with all activities of daily living, including showering, dressing, grooming,
toileting, mobility, feeding, taking medications,cleaning, and other duties as requested.
2. Assistresidents with managing home, activities; following goals on their service careplan.
3. Assistindividuals with self-administering medications.
4. Respondpromptly to individuals' requests for assistance.
5. Accuratelydocument provision of care as directed.
6. Stay currentwith changes in care/support (e.g. medications, treatments, behavior plans,etc.).
7. Alert programmanagement in the event of an individual's change in status or behavior.
8. Assist withweighing individuals, and gathers and records other data as requested.
9. Complete alldocumentation and reports within program timelines.
10. Actively participates in resident, family, team andstaff meetings.
11. Attend trainings as assigned maintain certificationof required trainings (CPR, First Aid, and Medication).
12. May possibly transport Clients as request in theirown vehicle or staff's own personal vehicle.
Qualifications:
  • A high school diploma or equivalent. One year experience providing direct carepreferably to individuals
  • Ability to understand, speak, read and writefluent English; sensitive to client's needs
  • Strongly prefer that a candidate will have ademonstrated understanding of and competence in serving culturally diversepopulations, can communicate effectively, use good judgment, high energy level, function well in a team atmosphere; perform basic computer functions
  • Must be able to perform each essential duty satisfactorily
  • Must have a valid driver's license and access toan operational and insured vehicle in addition to willingness to use it totransport residents
