Case Manager, Durango Integrated Healthcare, Durango, CO

Position Summary:ย We are seeking a Case Manager who will participate as a member of a treatment team to design and implement care plans for patients with a wide range of behavioral health conditions. Assists patients in accessing healthcare services and resources to meet healthcare needs, utilizing a trauma-informed, patient centered approach focusing on patient identified goals. Assists patients in improving overall quality of life through improvement in health, relationships, meaningful activities and access to resources.

Qualifications: Our ideal candidate for our Case Manager position will maintain a Bachelorโ€™s degree in health or human service-related field;.ย  Additionally, they will have a minimum of 1 year in health or human services setting; Preferredย 2 years case management experience. Additional Certifications:ย BLS, CPI

Salary: ย Starting at $21.00 (Base $18.50 + $2.50 Locale differential) - $23.72 per hour (non-exempt/hourly) and considers preferred experience and preferred education described above.

Physical demands: Employee must occasionally lift and/or move 15 pounds up to 50 pounds on an occasional basis. Employee must be able to travel between AHS locations. Possible potential exposure to communicable disease. The noise level in the work environment is usually moderate. Required Skills:

  • Must have current driverโ€™s license and ability to be commercially insured and maintain a driving record that allows insurability with AHS commercial insurance agency.
  • Advanced Microsoft computer skills required.

  • Must have excellent interpersonal skills and demonstrated ability to work effectively both independently and as part of a team.
  • High attention to detail is essential.
