Position Summary

We are looking for a compassionate, organized, and tech-savvy Consultative Care Coordinator to serve as a key liaison between patients and our Functional Medicine Hormone Pharmacy Consultants. This role is ideal for someone who enjoys helping patients navigate their care journey, can communicate clearly and empathetically, and is comfortable managing multiple tasks in a fast-paced, evolving healthcare environment.

The Consultative Care Coordinator will help triage patient needs, schedule appointments, coordinate communication, collect and manage health information, and support both patients and consultants with excellent attention to detail and professionalism.

Key Responsibilities

  • Serve as the primary point of contact for patients seeking support from Functional Medicine Hormone Pharmacy Consultants
  • Triage patient needs and determine appropriate next steps based on patient questions, concerns, and urgency
  • Schedule, confirm, and manage patient appointments with consultants
  • Communicate clearly and compassionately with patients via phone, email, text, or patient portal
  • Collect, organize, and document patient health information accurately and in a timely manner
  • Ensure required forms, lab results, medical history, and other relevant information are received before consultations
  • Coordinate communication between patients, pharmacy consultants, providers, and internal team members
  • Assist patients with technology-related needs, including patient portals, scheduling platforms, intake forms, and telehealth tools
  • Maintain confidentiality and follow all applicable privacy and compliance standards
  • Track deadlines, follow-ups, and outstanding patient information to ensure timely care coordination
  • Support workflow improvements and adapt to new processes, systems, and responsibilities as the organization grows
