Job description

  • Serve patients of all ages through skilled home care nursing visits
  • Coordinate care plans and provide patient and family education
  • Perform skilled nursing assessments and care
  • Coordinate and provide direct and indirect patient care using the nursing process
  • Perform case management activities including assessment, coordination, planning, monitoring, implementation, and evaluation
  • Interact with clients, caregivers, and families to assess needs, plan care, arrange services, monitor progress, and provide support and education
  • Complete admission documentation, OASIS/485 changes, visit notes, telephone records, physician orders, and discharge documentation according to policy timelines
  • Coordinate the healthcare team and initiate case conferences as necessary
  • Direct licensed practical nurse activities
  • Assign and supervise home health aides
  • Perform OASIS assessments for admissions, recertifications, resumptions, and discharges
  • Perform complex wound care, including wound vacs, packing, and staple or suture removal
  • Monitor central lines and perform dressing changes
  • Access mediports for IV infusion or blood draws and flushes
  • Communicate with physicians regarding patient updates and condition changes
  • Perform case management for assigned patients
Requirements
  • Current Illinois RN certification/licensure
  • BLS certification within 30 days
  • Valid driver's license
  • Proof of auto insurance
  • College diploma in nursing
  • At least 1 year of nursing experience
  • Knowledge of Medicare Home Care benefits and covered services
  • Knowledge of State and Federal regulatory and accreditation guidelines
  • Ability to perform OASIS assessments
  • Ability to provide complex wound care, including wound vacs
  • Ability to monitor PICC and Hickman catheters and perform dressing changes
  • Ability to access mediports for IV infusion or blood draws and flushes
  • Ability to complete required clinical documentation accurately and within policy timelines
