Job description

Hospice Registered Nurse (RN) โ€“ Field Visits Only


Location: Waukesha, Milwaukee, Washington, and Ozaukee Counties, WI

Schedule: Mondayโ€“Friday, 8:00 AM โ€“ 5:00 PM
On-Call: None
Hours: 40 hours guaranteed weekly
Patient Visits: Approximately 5 visits per day
Mileage Reimbursement: $0.53 per mile


Position Overview

We are seeking a compassionate and experienced Registered Nurse (RN) to provide hospice care throughout southeastern Wisconsin. This is a field-based position focused exclusively on patient visitsโ€”no case management responsibilities and no participation in IDG meetings.

The ideal candidate will have hospice and wound care experience, strong clinical assessment skills, and a passion for delivering high-quality patient-centered care in the home setting.


Responsibilities

  • Perform comprehensive assessments of hospice patients in their homes
  • Develop and implement individualized care plans in collaboration with providers and caregivers
  • Monitor patient conditions and identify opportunities for symptom management and illness prevention
  • Coordinate care transitions and communicate effectively with physicians, social workers, and other healthcare professionals
  • Provide patient and family education regarding treatment plans and disease progression
  • Advocate for patients while maintaining confidentiality and compliance with healthcare regulations
  • Maintain accurate and timely clinical documentation within the EMR system
  • Ensure quality outcomes through proactive intervention and ongoing reassessment


Required Qualifications

  • Active Registered Nurse (RN) license in Wisconsin
  • Hospice nursing experience
  • Wound care experience
  • Current CPR/BLS certification
  • Valid driver's license
  • Reliable transportation
  • Valid automobile insurance
  • Ability to travel throughout assigned counties


Preferred Qualifications

  • Experience with Homecare Homebase (HCHB)
  • Experience with MatrixCare or other Electronic Medical Record (EMR) systems
  • Associate's or Bachelor's Degree in Nursing
  • Case Management certification
