Job description

We are looking for an empathetic, organized, and driven master's Level Case Manager. In this role, you will assess client needs, design individualized care plans, and connect individuals with critical community resources. You will advocate for clients and help them navigate complex support systems. This position will work with adult/gero acute patients.

Key Responsibilities

  • Conduct detailed interviews to identify the psychological, medical, and social needs of each client.
  • Develop specific, step-by-step care plans to help clients reach their goals.
  • Provide individual, group, and family psychoeducation-oriented care using various professional treatment modalities.
  • Coordinate individualized discharge planning for patients with physician.
  • Communicate legal requirements for case management with physician and nursing personnel.
  • Document treatment provided to patient in medical record.
  • Report incidents of suspected abuse using prescribed forms and protocols from the relevant community/state agency.
  • Link patients to housing, food assistance, healthcare, and financial aid programs.
  • Document all client interactions and maintain accurate, confidential files.

Requirements

  • Masterโ€™s degree in social work (MSW), Psychology, Counseling, or a closely related health field required.
  • Minimum of two (2) years of professional case management or social work experience.
  • Compassionate with excellent communication and problem-solving skills.
  • Proficient with electronic health records (EHR) and basic office software.
  • Experience with therapeutic limit-setting helpful or an understanding of level systems or other behavior management modalities helpful.
  • Ability to maintain professional boundaries
