In this role, you’ll support patients with complex conditions by coordinating care, connecting resources, and empowering individuals to take control of their health.


???? Compensation

We offer competitive pay based on education, experience, and skills:

  • LPN: Starting at $56,160/year, commensurate with experience
  • RN (Associate Degree): Starting at $76,968.35/year, commensurate with experience
  • RN (Bachelor’s Degree): Starting at $78,200/year, commensurate with experience

???? Work Schedule
  • Monday through Friday
  • Choose between:
    • Five 8-hour shifts, or
    • Four 10-hour shifts
  • Scheduled within clinic hours of 7:00 AM – 6:00 PM

???? What You’ll DoCare Coordination & Case Management
  • Conduct comprehensive assessments of patient needs (medical, social, behavioral, financial)
  • Develop and implement individualized, cost-effective care plans
  • Collaborate with providers, behavioral health teams, and specialists
  • Coordinate referrals and community resources
  • Monitor services to ensure high-quality, efficient care delivery
Patient Advocacy & Support
  • Act as a trusted advocate for patients and families
  • Help patients navigate healthcare systems and overcome barriers
  • Build and maintain community partnerships
Education & Engagement
  • Provide disease management and preventive care education
  • Support patient self-management and goal-setting
  • Promote health across all age groups
Clinical & Administrative Excellence
  • Maintain accurate, confidential patient records
  • Track high-risk populations and outcomes
  • Support quality assurance and compliance efforts

  • Graduation from an accredited nursing program
  • Current Hawaiʻi LPN or RN license
  • Current BLS certification
  • Minimum 1 year of nursing experience
Preferred
  • Community health or managed care experience
  • Experience working with diverse or underserved populations
