As a Care Partner in our Terrace Club neighborhood, you provide personalized, relationship-based care to a consistent group of residents, helping create meaningful and successful days.
What You'll Do:
• Provide hands-on care and emotional support based on each resident's Individualized Service Plan (ISP)
• Partner with residents through prompting, cueing, and reminders
• Observe, document, and report changes in condition
• Support daily programs and meaningful engagement activities
• Assist with dining, housekeeping, and laundry services
• Promote safety, dignity, and Resident Rights at all times
• Communicate regularly with families and care leadership
• Follow infection control, OSHA, and safety standards
You serve as a trusted advocate and consistent presence, building strong relationships that enhance quality of life.
What We're Looking For:
• High School Diploma/GED; age 18+
• Experience working with seniors preferred
• Compassionate, patient, and dependable
• Strong communication and organizational skills
• Ability to remain calm and make sound decisions
• CNA/HHA certification (as required by state, NJ required)
