Description

Direct care companion provide support to patients through companionship and socialization as well as respite relief for caregivers. They may also make tuck in calls to patients and families in accordance with their care plan. Gain hands on experience in patient care, while making a difference in the lives of others. Hospice volunteers are a valuable part of our care team.

Qualifications

The patient care companion must possess the ability to actively listen and communicate compassionately with patients, families and caregivers. They must be able to work well as apart of our interdisciplinary team. The volunteer must possess emotional maturity and sensitivity while also respecting the beliefs of the patients and their families. Volunteers must be dependable and have reliable transportation to ensure they are able to complete scheduled visits. The volunteer must have an understanding of the hospice philosophy, goals and be willing to participate in continuing education requirements.

Responsibilities

Provide support to patient and families by performing such essential task as: reading to patients, playing games, writing letters on behalf of patients, assisting with meal preparation, light household chores as needed, companionship, and caregiver respite relief.

Participate in continuing education and in-service learning as required by the agency.

Provide availability on a regular basis and/or ensuring that visits with the patient are completed as outlined on their care plan.

Maintain regular communication with the Volunteer Coordinator (VC) about the needs of the patient, family or caregiver and report any concerns/ problems in a timely matter to VC.

Document and record all hours or service and return volunteer visit form to VC with 72hr of all visits.

Notify VC if there are any changes that could affect patient care, a change in status, leave or absence or need or a substitute volunteer.

Maintain patient, family and caregiver confidentiality and abide by all state and federal regulations as established in the company's policy and procedures.

Meet and exceed the delivery of Company service standards and operate under the company's code of conduct.

Perform duties within the patient's established volunteer care plan according the individuals ability, knowledge and training. Do not assume roles or responsibilities outside of the scope of the volunteer training practices.
