Position Overview

Heart Share Community Services is seeking a dedicated and empathetic Licensed Clinical Social Worker (LCSW) to join our team as a full-time remote clinician. This role is pivotal in providing psychosocial support and therapeutic services to clients within our Hospice Center, addressing the complex needs of individuals with major mental illnesses and severe emotional disorders. Key Responsibilities
  • Provide direct clinical support to clients both remotely and in the community, specifically focusing on those experiencing major mental illnesses and severe emotional disorders.
  • Develop and implement integrated care plans that ensure clients' needs are addressed through effective communication and collaboration with families and external service providers.
  • Engage in the assessment process by conducting brief, evidence-based screenings and arranging for comprehensive evaluations as needed.
  • Monitor and manage a caseload that includes individual, group, and family therapy sessions, ensuring that treatment responses are centered around clients' presenting issues.
  • Participate actively in crisis intervention efforts and provide timely support when needed.
  • Keep accurate records and documentation of client interactions, ensuring timely completion of reports and data submissions.
  • Take part in organizational committees aimed at enhancing service delivery and quality improvement initiatives.

This role emphasizes working with a diverse clientele, many of whom are located in underserved communities. Qualifications
  • A valid and active LCSW license in the state of Florida.

  • A demonstrated commitment to maintaining ethical standards and fostering a respectful environment for people from diverse backgrounds.
  • Organizational and planning skills that facilitate effective time management and treatment delivery.
  • Competency in age-related issues and developmental needs for different populations.
Preferred Experience
  • A minimum of 2 years of clinical experience in social work or mental health settings is preferable.
  • Experience in hospice or palliative care settings will be a significant advantage.
  • Familiarity with care coordination processes and integrated care models.
Compensation and Benefits

This full-time position offers a competitive salary range of USD 70,000 to 85,000 annually.
