Position Overview

We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) for a full-time position within our remote outpatient setting, specifically focusing on our Hospice Center clientele. This role involves providing high-quality mental health services that support individuals in navigating their psychological and emotional challenges.

Work Environment

As a fully remote position, our Licensed Clinical Social Worker role allows for flexible scheduling with opportunities to accommodate evening and weekend hours when necessary. You will work with a supportive team committed to excellence in patient care regardless of location. Our Hospice Center provides a unique setting where your expertise will help both clients and their families through critical transitions.

Key Responsibilities
  • Conduct thorough psychosocial assessments to understand clients' mental health concerns, strengths, and resources essential for effective treatment planning.
  • Deliver evidence-based individual, family, and group therapy utilizing various modalities such as cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-informed care.
  • Collaboratively develop and implement personalized treatment plans, setting achievable objectives that reflect clients' unique circumstances.
  • Engage actively with a multidisciplinary team, including psychiatrists and psychologists, to coordinate care and ensure a holistic approach to the treatment process.
  • Provide timely crisis intervention, risk assessments, and safety plans that are responsive to clients' immediate needs.
  • Maintain accurate and comprehensive documentation to adhere to ethical and legal standards while ensuring confidentiality and respect for clients.
  • Facilitate group therapeutic sessions, psychoeducation classes, and support groups aimed at building community and resilience among clients.
Patient Population / Client Focus

This role primarily focuses on a diverse client population within a Hospice Center. You will support individuals coping with serious illness and end-of-life issues, addressing mental health concerns such as grief, anxiety, trauma, and adjustment disorders. Your work will significantly enhance their quality of life and emotional wellbeing during challenging times, allowing clients and families to navigate transitions more smoothly.

Qualifications
  • A Masterโ€™s degree in Social Work from an accredited institution.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW) in Georgia.
  • Minimum of two years of experience working in a mental health or outpatient setting, showcasing capability to handle the complexities of clinical social work.
  • Strong foundation in evidence-based treatment methodologies and the ability to tailor approaches to individual client needs.
  • Exceptional interpersonal and communication skills, with a focus on building rapport and trust with clients and their families.
Preferred Experience
  • Experience in hospice, palliative care, or bereavement counseling, enhancing your effectiveness in this specialized client population.
  • Familiarity with electronic health record (EHR) systems and telehealth platforms to streamline client interactions and documentation.
