Working under the direction of the Social Work Supervisor, the CRS assists the SW team by connecting resources to meet patients' needs for Medical/Surgical, Emergency Department, Perinatal, and Oncology services.Job Description:
Essential Responsibilities:
Determines, either independently and/or in conjunction with clinicians, the resources that would optimally meet patients' and families' needs and make independent decisions about what services to recommend to a patient/family. Maintains a caseload with multiple patient contacts per day, including phone calls, paperwork, and face-to-face contacts. Researches best practices and programs to meet the needs of patients and functions as the primary health care contact for psychosocially complex patients when needed. Provide timely and appropriate documentation in patients' medical records using EPIC electronic medical records. Advocate on patients' behalf in written and verbal communication. Assists patients and families with such tasks as accessing entitlement programs. Provides appropriate medical information as requested by extended care facilities, Department of Transitional Assistance, etc. Conduct staff and community-based training on resources. Provides updates to staff on procedural requirements and changes required by state and local agencies. Documents and advises supervisor and clinical staff of obstacles to obtaining services that require social action. Develops liaisons with community and governmental agencies to facilitate the negotiation of complex systems. Complete Mass Health applications for the needed patient population. Coordinates community initiatives such as the car seat program and coat drives. Participates in making and managing schedules including on-call schedules. Oversees onboarding and offboarding processes for the department. Assist with other department tasks as assigned.
Required Qualifications:
High School diploma or GED required. Bachelor's degree preferred in Social Work or Human Services
License preferred: Licensed Social Worker
1-3 years related work experience required.
2 years of related work experience without a related Bachelor's degree
Experience with computer systems required, including web based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access.
Competencies:
Written Communications: Ability to communicate clearly and effectively in written English with internal and external customers.
Oral Communications: Ability to comprehend and converse in English to communicate effectively with medical center staff, patients, families and external customers.
Knowledge: Ability to demonstrate full working knowledge of standard concepts, practices, procedures and policies with the ability to use them in varied situations.
Team Work: Ability to work collaboratively in small teams to improve the operations of immediate work group by offering ideas, identifying issues, and respecting team members.
Customer Service: Ability to provide a high level of customer service to patients, visitors, staff and external customers in a professional, service-oriented, respectful manner using skills in active listening and problem solving. Ability to remain calm in stressful situations.
Age based Competencies:

Physical Nature of the Job:

Pay Range:
$57,200.00 USD - $76,982.88 USDThe pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
