Job description

The Community Health Coordinator will support the implementation and coordination of community-based health programs, including the Live to the Beat initiative, focused on improving health outcomes in communities around Atlanta. The Coordinator will deliver culturally responsive health education and participant support, facilitate program activities and partner engagement, and assist individuals in navigating healthcare and social care resources. The role also includes data tracking, reporting, and collaboration with internal and external partners to support program evaluation, continuous improvement, and community outreach. This is a full-time position up to 37.5 hours per week with the American Heart Association. You will be a direct employee of the Association, working an embedded within a clinical partnership in our region. The position is grant-funded through June 30, 2027.The position is hybrid with the expectation that you will spend the majority of your time in the community. You will report regularly to the Association or the clinical partner site for team meetings, training, and huddles.

Responsibilities- Promote health education and health literacy in underserved communities through culturally responsive communication and outreach.
- Connect patients to care teams and community resources; follow up to ensure needs are met.
- Facilitate implementation and ongoing refinement of community-based health programs, including the Live to the Beat powered by Wellstar initiative, aligned with program goals of improving heart health through sustainable behavior change.
- Deliver participant-centered health education and monitoring activities, including support for understanding health conditions, developing improvement strategies, and reinforcing healthy lifestyle habits (e.g., blood pressure self-monitoring, nutrition, physical activity).
- Provide informal participant support, follow-up, and navigation assistance, including connecting individuals to a medical home, primary care services, and social care resources (e.g., Wellstar Find Help), addressing barriers to care with empathy and consistency.
- Coordinate program activities and partnerships, including monthly class sessions, partner check-ins, meetings, and site visits with congregational sites, internal teams, and external stakeholders.
- Prepare and distribute program materials, supplies, curricula, and participant tools (e.g., home blood pressure monitors and educational resources) to support effective session delivery and engagement.
- Track participant engagement, attendance, biometrics, and other program indicators; monitor and report outcomes, expenses, and implementation progress to support evaluation and continuous improvement.
- Support the development and maintenance of a resource repository focused on social determinants of health for youth and families in the focus area.
- Participate in required training (including Community Health Worker training and supplemental role development) and apply approved curriculum, referral protocols, and engagement strategies in daily work.
- Collaborate with partners and program leaders (e.g., American Heart Association, Wellstar, ARCHI) through meetings, calls, site visits, and feedback processes to strengthen alignment, identify improvement opportunities, and advance shared goals.
- Contribute to program communications and reporting by drafting summaries, sharing participant insights and key data, and presenting information in clear, accessible, and culturally appropriate formats tailored to multiple audiences.
- Maintain trusted, professional, and relationship-centered engagement with participants, congregational partners, and community stakeholders, demonstrating reliability, cultural humility, and responsiveness.
- High school diploma or equivalent and 3 years minimum of prior related experience.
- Progressive experience in public health, community health or equivalent
- Proven ability to recruit, train, mobilize and manage volunteers
- Demonstrated ability to drive toward and achieve results
- Effectively manage relationships with the ability to gain trust and demonstrate influence without authority
- Knowledge of community networks
- Completion of a Community Health Worker training or willingness to complete one.
- Proficiency in Microsoft Office applications
- Ability to reliably travel to meeting/training sites and availability to coordinate schedules with participating organizations and volunteers, including outside of regular business hours (weekends/evenings). This position will require the Coordinator to attend occasional Saturday events.
Here are some of the preferred experiences and skills we are seeking:
- Knowledge of the social determinants of health (SDOH) as it relates to behavior modification through self-care activities and policy/system-wide changes.
- Understanding of local health systems, relationships with hospitals, health clinics, and Federally Qualified Health Centers preferred.
- Bilingual or multilingual - English and Spanish, preferred to reflect communities served.
