Job description

POSITION SUMMARY: The Community Relations Assistant works in conjunction with the other members of the Admission Team and is responsible for activities and actions related to promoting the services and expertise of the organization to the medical community and to patients and family members.
REPORTS TO: Community Relations Liaisons, Director of Community Relations
SUPERVISES: None
QUALIFICATIONS:
Education: High School Diploma
Credentials: CNA or CHHA certification preferred
Experience: At least one year of healthcare experience is preferred. Bilingual preferred.
Core Competencies: Knowledge of company requirements including Medicare, Medicaid, and private insurance guidelines. Familiar with Microsoft Excel and Word program. Good communication and marketing skills. Ability to respond to the needs of physicians, discharge planners, patients, and family members. Ability to make meaningful observations and write comprehensive accurate reports.
FUNCTIONS & RESPONSIBLITIES:
1. Work in conjunction with the admission team.
2. Goes in to the field and establishes presence of our organization to our service members.
3. Responds to referrals for evaluation and/or admission to the organizationâ€'s program.
4. Explains the program to patients, family members, and other referral sources.
5. Works with admission team for referrals, admissions, and care coordination of patient and family needs.
6. Identifies key referral sources and assists the admission counselors to develop a marketing plan for the program.
7. Attends and participates in in-services, staff meetings and case conferences.
8. Completes all documentation in an accurate and legible manner.
9. Complies with company policies and procedures, laws and regulations and standards of practice.
10. All other tasks and duties deemed necessary and appropriate.
