Work Environment

This role is exclusively remote, allowing for flexibility while ensuring you can maintain close contact with clients through virtual consultations. Despite the remote environment, you will be an integral part of our interdisciplinary team, collaborating with nurses, caregivers, and other health professionals to ensure a comprehensive approach to client care. The full-time work schedule is Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evenings or weekends required based on client needs. On-call availability may also be necessary to provide instant support in urgent situations.

Key Responsibilities

As a Licensed Clinical Social Worker, your responsibilities will include:

  • Conducting thorough assessments of seniors' psychosocial needs, considering their emotional, mental, and social factors.
  • Developing personalized care plans in collaboration with clients and their families to address specific goals and needs.
  • Providing counseling and emotional support to help seniors navigate life transitions, such as health changes or loss of loved ones.
  • Advocating for clients' rights and facilitating their access to essential community resources to ensure they receive appropriate services.
  • Coordinating care with interdisciplinary teams, enhancing the collaborative approach to client health and well-being.
  • Regularly monitoring clients' well-being through virtual visits to assess living conditions and address potential safety concerns.
  • Educating clients and families about available support groups and community programs to foster social engagement.
  • Maintaining accurate documentation of client interactions, assessments, and care plans in compliance with ethical and regulatory standards.
Patient Population / Client Focus

Your primary focus will be on supporting seniors, a population that often faces complex physical and emotional challenges as they navigate aging. You will work closely with clients and their families, providing tailored support to help them maintain dignity and high quality of life. Your role is crucial in addressing their unique needs, ensuring they have access to appropriate mental health resources and social services.

Qualifications
  • Master's Degree in Social Work from an accredited institution.
  • Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of Texas.
  • Minimum of two years of experience in a social work position, ideally within a community health clinic or geriatric setting.
  • Demonstrated knowledge of social work principles, aging-related issues, and existing community resources.
  • Exceptional assessment, counseling, and problem-solving skills.
  • Strong ability to establish rapport with seniors and their families through empathy and compassion.
  • Proficient in electronic health record (EHR) systems and standard computer applications.
Preferred Experience
