Since 1995, Christ Community Health Services (CCHS) has provided high-quality healthcare to the underserved in the context of distinctively Christian service. As a faith-based Federally Qualified Health Center (FQHC), we serve the medically underserved of Memphis, TN, offering primary care, dental, behavioral health, pharmacy, and specialty services to the communities that need them most.

About the Role

The LPN Team Lead provides nursing care to CCHS patients, assisting physicians and nurse practitioners with various aspects of patient care and performing patient testing as ordered. The LPN helps ensure smooth patient flow through the clinic and also assists with clerical responsibilities.

As Team Lead, this role also serves as a working lead for other LPNs on the care team — providing daily work direction, training, and competency validation. (Formal supervisory authority, including discipline, rests with the LPN/RN Clinical Supervisor.)

What You'll Do

  • Assist with staff education and supervision, such as precepting new LPNs.
  • Assist as needed with same-day referral requests.
  • Implement and maintain morning team huddles as part of our PCMH, team-based care model.
  • Assist as needed with initiation of medical history and physical assessment; draw blood and collect other specimens for lab analysis; prepare for and assist with medical procedures such as GYN exams and wound care.
  • Provide telephone triage and initiate follow-up calls as directed by the physician or clinic charge nurse; document call outcomes in the medical record.
  • Educate patients and families within your scope of knowledge, providing informational material as appropriate.
  • Complete medication refill tasks as needed for your assigned care team.
  • Follow up on the STD tracking report and on patients with abnormal labs for your assigned care team.
  • Oversee the VFC (Vaccines for Children) program.
  • Assist with quality improvement activities as directed.
  • Clean exam and treatment rooms following patient discharge, and ensure patient care equipment is clean and properly functioning, reporting defects to your supervisor.
  • Perform daily quality control checks on clinic equipment and document results in log records.
  • Uphold and implement Nursing standing orders and protocols.
  • Maintain adequate levels of supplies, medications, linens, and equipment, requisitioning more as needed.
  • Respond to inquiries and communicate policies, procedures, and standards to patients, families, visitors, and fellow employees.

What You'll Bring

  • High school diploma plus specialized nursing training.
  • Minimum of one year of nursing experience.
  • Current LPN Tennessee.
  • Current BCLS certification.
  • Comfort providing daily work direction, training, and competency validation to fellow LPNs as a working team lead.
