Job description

About the Role 

As the Associate Outreach Leader Associate, you will serve as a key member of Hopscotch Primary Care's Outreach team, supporting patient growth and community engagement efforts across assigned clinics and territories. This role combines community outreach, event coordination, grassroots relationship management, and prospective patient engagement to help increase awareness of Hopscotch Health and connect eligible patients to care. 

This role plays a highly visible role within the community and serves as a trusted ambassador of the Hopscotch brand. You will build relationships with local organizations, support community events and clinic open houses, educate prospective patients about Hopscotch services, and help guide individuals through the patient enrollment journey. 

This role works collaboratively alongside Outreach Leaders (OL), clinic teams, marketing, and patient engagement teams to maximize community presence and support patient acquisition efforts. The Associate Outreach Leader is not a direct report to the OL; however, the two roles work closely together to align outreach activities, coordinate territory coverage, and support shared growth goals. 

This role is ideal for someone who enjoys building relationships, engaging with seniors and community organizations, coordinating events, and helping patients navigate healthcare access in a fast-paced and mission-driven environment. 

Key Responsibilities 

Community Outreach & Relationship Building 

  • Build and maintain relationships with grassroots community organizations - senior centers, housing communities, faith-based organizations, nonprofits, community centers, and local civic groups not currently managed by the OL 
  • Represent Hopscotch Primary Care professionally within the community and serve as a trusted point of contact for prospective patients and community stakeholders 
  • Educate community members on Hopscotch services, providers, and the value-based care model, and conduct outreach activities to increase awareness and engagement 
  • Maintain recurring touchpoints with community organizations to support long-term engagement and collaboration 

Community Events & Open Houses 

  • Co-lead, coordinate, or independently support outreach events including health fairs, educational seminars, senior community visits, and grassroots engagement opportunities 
  • Support planning and execution of clinic open houses, including venue preparation, outreach materials, staffing coordination, setup, and follow-up activities 
  • Greet and engage prospective patients during events, capture attendee information, and conduct timely follow-up to support conversion 
  • Conduct on-site enrollment activities or schedule follow-up appointments when appropriate, and provide post-event reporting on attendance and outcomes 

Patient Engagement & Growth Support 

  • Actively identify and engage eligible Medicare and Value-Based Care (VBC) patients within assigned communities through calls, texts, email, and in-person interactions 
  • Assist prospective patients with scheduling appointments and navigating the enrollment process 
  • Conduct timely follow-up with prospective patients generated through outreach, events, referrals, and inbound inquiries to improve conversion and retention 
  • Support patient onboarding and re-engage inactive or unresponsive prospective patients when appropriate 

Outreach Coordination & Collaboration 

  • Coordinate outreach calendars, event schedules, and community activities with Outreach Leaders and internal teams to maximize coverage and minimize duplication 
  • Share event feedback, community insights, and outreach learnings during regular collaboration meetings 
  • Partner with clinic teams, marketing, and patient engagement teams to support outreach campaigns and patient growth initiatives 

Operational & CRM Excellence 

  • Track outreach activities, event participation, patient engagement efforts, and follow-up outcomes 
  • Enter and maintain accurate documentation within Salesforce and other business systems 
  • Maintain outreach calendars, event preparation timelines, and outreach inventory/materials, and support reporting and operational readiness

Qualifications

  • Required: high school diploma or equivalent, valid driver's license, U.S. work authorization, ability to travel locally throughout assigned territory, and comfort using Microsoft Office and CRM systems 
  • Preferred: 1-3 years of healthcare outreach, community engagement, sales support, event coordination, or customer service experience 
  • Preferred: experience working with Medicare or senior populations, event planning or community engagement experience, and CRM/Salesforce experience
