Job Description:
Community Health Workers (CHW's) at NovumHealth are trusted members of the community who help individuals navigate challenges with mental health, substance use, and social barriers. CHWs meet people where they are, whether at home, in the community, or in recovery settings. Building trust, providing support, and connecting members with care and resources.
Job Specific Duties:
  • Assessing the healthcare needs of individuals within the community and identifying gaps in care.
  • Collaborating with healthcare providers, social service agencies, and community organizations to develop comprehensive care plans for patients.
  • Assisting patients in navigating the healthcare system and accessing necessary services such as medical appointments, specialist referrals, diagnostics tests, and community resources.
  • Conducting home visits or outreach activities to assess patients' living conditions, social support systems and adherence to treatment plans.
  • Provide transportation, with personal vehicle, for members to appointments when needed (mileage is reimbursed).

Required Education, Experience, Skills & Abilities:
  • An active and valid Community Health Worker Certification.
  • At least 1 year of experience in community health, behavioral health, peer support, or related roles.
  • CPR/First Aid (Or willingness to obtain).

Physical Requirements:
  • Ability to sit, use hands and fingers, talk or hear, and smell continually.
  • Close vision is required to see computer monitors, read documents, and operate copy and fax machines.
  • Ability to stand, walk, and reach frequently.
  • Must be able to move quickly in emergencies, including walking briskly or running short distances.
  • Ability to maintain physical and mental alertness during prolonged periods of activity in high-stress environments.
