Oregon City, Gladstone, Damascus, OR Area

Minimum Qualifications:

  • Must be a licensed Registered Nurse in the state of Oregon

Preferred Qualifications:

  • Experience working in Home Health

Responsibilities:

  • Develop care plans for the patient
  • Complete initial assessment for care of the patient
  • Provide case management from admission to discharge
