The Community Health Educator Screening RN will participate in health fairs and other community events to provide health resources, education, and/or clinical screenings. This role is responsible for all aspects of event execution, from preparation and transport of materials to data collection and follow-up.
Qualifications
Education, Knowledge, Skills and Abilities Required: Education Required:
  • Associate's in Nursing.
  • Proficient with standard office and medical screening equipment.
  • Ability to work effectively both independently and as part of a team.
  • Excellent written and verbal communication skills.
  • Proficient computer skills that include but are not limited to Google Suite and/or Microsoft Office platforms.

Education, Knowledge, Skills and Abilities Preferred:
  • Bachelor of Science in Nursing (BSN).
  • Experience in community health education or a similar role.
  • Bilingual skills.

Licenses and Certifications Required:
  • NJ State Professional Registered Nurse License.
  • Valid Driver's License from a USA state.
  • BLS AHA Basic Health Care Life Support HCP Certification

Responsibilities
A day in the life of a Community Health Educator Screening RN at Hackensack Meridian Health includes:
  • Coordinates and conducts community health education programs on a variety of topics, including preparing written curricula, program descriptions and other materials as needed.
  • Conducts health screenings in the community, such as blood pressure, cholesterol and glucose screenings, in a technically competent and timely manner.
  • Transports, assembles, and breaks down screening equipment, literature and exhibit tables for community events.
  • Provides education to community members to help them understand screening results and offers referrals for follow-up care as needed.
  • Accurately collects, inputs, and maintains data in the outreach database or other systems, providing summaries and evaluation forms as required.
  • Obtains signed consent forms for screenings and ensures their proper handling and safekeeping.
  • Oversees per diem staff and/or medical students assisting with screenings and educational events, providing direction as needed.
  • Maintains an adequate inventory of medical and general supplies for outreach activities, if as applicable.
  • Responds to inquiries from the call center and community members regarding classes, services, and health information in a timely manner, if applicable.
  • Identifies and maintains relationships with community organizations as directed by the manager, if applicable.
  • Other duties and/or projects as assigned.
  • Adheres to HMH Organizational competencies and standards of behavior.

Starting Minimum Rate
Minimum rate of $51.90 Hourly
