Job description

Overview
The Community Health RN provides direct community health services through Care Connect to vulnerable populations through identification, engagement, and provision of resources to address unmet social and health needs.
Responsibilities
Provides clinical support for the delivery of UF Health St John's Mobile Health Services.
Engage clients to identify and remove social barriers to managing their health.
Develop and implement community health improvement programming in response to community needs identified through St. Johns County Community Health Needs Assessment and Community Health Improvement Plan.
Establish positive relationships with community based organizations and other key partners to gain understanding of the physical, social, and economic needs of the community.
Map community based assets and collaborates with key stakeholders to leverage available resources for development of new and accessibility to existing health education/promotion programs.
Serve as liaison to community organizations aimed at improving the health and well-being of the region.
All other duties as assigned by department.
Qualifications
Required education:
  • ASN

Required experience:
  • 2 years of RN experience

Preferred experience:
  • Experience working with vulnerable populations

Required license/certifications and others:
  • RN license in the State of Florida
  • BLS
  • Complete level 2 background screening at hire
  • Motor Vehicle Record background screen at hire with proof of auto insurance
