Position Overview

Empower Health NP is seeking a dedicated and passionate Nurse Practitioner to join our team in Yonkers, New York. This full-time role offers an opportunity to work directly with patients in their homes, providing holistic care that addresses both acute and chronic health needs. Work Environment

This position is based onsite, providing you the opportunity to engage with patients in the comfort of their homes. You will work alongside a multidisciplinary team committed to delivering comprehensive health services in a supportive and empowering environment. Our regular work hours are from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Participation in community outreach events and health fairs may also be required on weekends. The average workweek encompasses approximately 40 hours.

Key Responsibilities
  • Conduct thorough health assessments and screenings for individuals and families to determine their comprehensive healthcare needs.
  • Diagnose and manage a wide range of acute and chronic medical conditions, implementing preventive care measures and health promotion strategies.
  • Partner with community organizations and stakeholders to develop and deliver health education programs that address specific community needs.
  • Provide culturally sensitive care and advocacy to ensure that all patients have access to the healthcare services they need.
  • Coordinate referrals and follow-up care to facilitate continuity of care for patients transitioning between services.
  • Maintain meticulous medical records in compliance with state regulations and industry best practices.
  • Engage actively in community outreach initiatives, including health fairs and wellness screenings, to increase community awareness of health resources.
  • Stay abreast of current trends, best practices, and policies in community health to inform your practice.
Patient Population / Client Focus

As a Nurse Practitioner at Empower Health NP, you will work primarily with diverse populations in a community setting. You'll encounter individuals and families facing a variety of health challenges, which may include managing chronic illnesses, addressing mental health concerns, and promoting overall wellness. Your efforts will directly impact the community's health by bridging gaps in healthcare access and promoting education.

Qualifications
  • A Bachelor's Degree in Nursing is required; a Master's or Doctoral degree in Nursing from an accredited program is preferred.
  • Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from recognized certifying bodies (ANCC or AANP).
  • A current, unrestricted Nurse Practitioner (APRN) license in New York State is mandatory.
  • Possession of a valid DEA# and NPI# is required for practice.
  • Strong clinical assessment and diagnostic abilities are essential for this role.
  • Excellent communication and interpersonal skills to foster strong relationships with patients and collaborate effectively with team members.
  • Ability to work independently and collaboratively in a team environment, demonstrating problem-solving skills and adaptability.
  • Proficient in electronic health records (EHR) systems for efficient patient documentation and management.
Preferred Experience
  • Minimum of 3-5 years of clinical experience, with a preference for those with experience in primary care or family medicine, particularly in community health or public health settings.
  • A demonstrated history of working effectively with diverse populations and understanding cultural competency are highly valued.
Compensation and Benefits

This position offers a competitive salary range of USD 150,000 โ€“ 160,000 annually, reflective of your experience and qualifications. In addition to salary, we provide a comprehensive benefits package that includes health insurance, retirement plans, and a generous sign-on bonus of up to $10,000.
