Behavioral Health Facilitator – PRNAs Needed | Centerville, Iowa | Travel to Other Locations as NeededBe the Connection That Makes Care Possible

Sometimes the most important part of a patient's healthcare experience is having someone there to make everything come together, someone who welcomes them, answers questions, coordinates with their provider, and helps ensure nothing falls through the cracks.

At Infinity Health, our Behavioral Health Facilitators are that connection.

We're looking for a compassionate and dependable CNA, Medical Assistant, or LPN to join our Behavioral Health team in a PRN (as-needed) position based primarily in Centerville. This role will also provide coverage at other Infinity Health locations as needed.

What You'll Do

As a Behavioral Health Facilitator, you'll work closely with our ARNPs, Psychiatrists, and care teams to create a positive and coordinated experience for our patients. You'll:

  • Welcome, room, and prepare patients for appointments
  • Facilitate TeleMed behavioral health visits
  • Obtain vital signs, reconcile medications, and update patient charts
  • Schedule in-person and virtual appointments
  • Administer injections as permitted by license or certification
  • Respond to patient calls and relay information to providers
  • Complete patient follow-up and engagement activities
  • Perform drug urine screenings and coordinate outside lab orders
  • Complete medication prior authorizations
  • Travel to other Infinity Health locations to provide coverage as needed

Who We're Looking For

We're looking for someone who is compassionate, dependable, organized, and comfortable working with patients receiving behavioral health services.

Candidates must have one of the following:

  • Certified Nursing Assistant (CNA)
  • Medical Assistant (MA)
  • Licensed Practical Nurse (LPN)

You'll also need strong communication skills, attention to detail, comfort with technology, and a willingness to support patients and teammates wherever you're needed.


Location & Schedule

Primary Location: Centerville, Iowa
Schedule: PRN / As Needed
Travel: Coverage at other Infinity Health locations may be needed
