About the Job
Job Summary:
MedStar Health is looking for a Community Health Program Coordinator to join our team! The ideal candidate will be a lay facilitator with teaching skills to promote healthy behavior change.
The Community Health Program Coordinator will coordinate community health programming and assist with the direction of community-based health resource centers supported by MedStar Health. Incumbent will work alongside regional Care Transformation team to coordinate operations related to center programming and health navigation including planning, communication, logistics, and documentation. Will work closely with partners across the organization, system, and community including government agencies, faith-based institutions, and non-profit organizations.

Primary Duties:
  • Serves as a liaison between communities, individuals and coordinated health care organizations. Builds and maintains relationships with community partners. Maintains program equipment and wellness areas to provide a safe, clean, and secure environment for members.

  • Communicates to clients the purposes of the program and the impact it may have on their wellbeing. Helps clients identify socio-economic issues/SDOH that affect or contribute to their overall health and supports with connection to existing community resources to meet these needs as available. Assists clients with healthcare navigation, including locating primary care providers, finding specialists, and attending outpatient appointments.

  • Identifies and addresses issues that create barriers to care for specific individuals. Provides culturally and linguistically appropriate health education. Collects, tracks and reports program data to community site Director and to MedStar Health.

  • Assists with the development of monthly programming schedules, volunteers' schedules, and room scheduling. Assists site director and MedStar Care Transformation team with designing, implementing, and evaluating health-related curricula appropriate to respective programs.

  • Coordinates and schedules meetings, conferences, classes, seminars, community activities and other related activities for the department. Ensure meeting readiness to include but limited to room availability, invitations, meeting requests and minutes, and communications. Assists in preparation to include agendas and minutes. Assists in the selection, training, orienting and assignment of volunteers in the health Center and its various programs. May assist with development of standards of operations and performance management planning.

Qualifications:
  • Bachelor's degree.

  • 2 years related experience.

  • Must obtain lay facilitator certification within 6 months of hire.

  • Certification as lay facilitator in health, wellness, fitness preferred.

  • Participation with a community health organization. experience as a lay facilitator in health/wellness programs preferred.

This position has a hiring range of
USD $20.57 - USD $36.27 /Hr.
