Starting Hourly Pay: $21.16

The Community Health Worker - Tribal Practices provides culturally relevant education on prevention and the promotion of holistic wellness for Indigenous people residing in Maricopa County. Additionally, this position is a liaison between the NATIVE HEALTH Community Health and Wellness Department, Medical Department, and community members to facilitate access to services to improve diabetes-related health outcomes. The CHW reports directly to the Indigenous Wellness Coordinator.

KNOWLEDGE, SKILLS, AND ABILITIES

1. Facilitate group classes proficiently using a wide range of skills and techniques to help participants understand their common objectives and assist them to achieve them.
2. Understand how social determinants of health impact the community and participants.
3. Knowledge of Arizona Indigenous tribes' practices of framing, gardening, and traditional foods.
4. Knowledge of client confidentially and client rights according to Health Insurance Portability and Accountability Act (HIPPA).
5. Ability to pass blood pressure and blood glucose testing competencies.
6. Strong problem-solving skills and assertive in challenging situations.
7. Highly resourceful team-player, with ability to also be effective independently. 
8. Creative thinker, who actively seeks opportunities and proposes solutions.
9. Demonstrates ability to write clear and concise reports of service activities and other programmatic reports.
10. Strong interpersonal skills including tact and flexibility to work effectively with supervisor, staff, and participants.
11. Advanced computer skills included MS Office, specifically EXCEL, PowerPoint, Outlook, and Word. 
12. Develop creative ideas, manage, and deliver diabetes-related education via digital content using a range of techniques, across channels including websites, social media, and Zoom platform.
13. Culturally competent; able to work with diverse workforce and client popluaiton. 

 QUALIFICATIONS/EXPERIENCE/REQUIREMENTS:

1.  A bachelor’s degree in a health-related field is required.
2. Experience in community outreach or health education is a plus.
3. Knowledge of Electronic Health Records (EHR) preferred, Athenahealth a plus.
4. Must be able to work irregular hours to include evenings and weekends as needed.
5. Must possess two years of specialized experience working within the American Indian/Alaska Native Community.
6. Must possess a valid and unrestricted Arizona driver's license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five (5) years. No more than two at-fault accidents in the last three years. Must maintain a valid Arizona driver's license during employment. Must be insurable under NATIVE HEALTH's liability auto policy. 
7. Must pass a criminal background check and obtain a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment and retain a valid card while employed with NATIVE HEALTH.
8. Must pass substance abuse testing upon employment and submit to random testing during employment. 
9. Must provide an updated immunization (IZ) record at the time of employment.
10. Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an annual update.
11. Must obtain Basic Life Support (BLS) certification within ninety (90) calendar days of employment and maintain certification during employment.
