Job description

Habilitation Aide – ICF Commons

Clearbrook has four Intermediate Care Facilities (aka ICF’s), each housing multiple clients. Goals are developed for each client based on their needs. Programming is assigned, usually pertaining to areas such as self-care, academics, community living & integration, as well as various workshop skills. Each facility has a nurse on staff at least 20 hours per week to coordinate any medical needs required by the clients.

Location:             Rolling Meadows

Hours:                  Part-Time/Full-Time

Schedule:            AM, PM, Weekend

Pay:       $21.35/hour + Paid Training!

Weekend Pay:  + $3/hr differential

Responsibilities:

  • Responsible for the direct provision of active treatment to the clients including training and assistance in all areas addressed in the individual program plans of the clients to whom she/he is assigned.  
  • Teach and provide the support necessary for the individual’s use of community resources. 
  • Assist clients with daily living activities including (but not limited to) personal hygiene, feeding & housekeeping. 
  • Assist clients in all areas of social and recreational program activities for the day.

Requirements:

  • 8th grade diploma or equivalent
  • Valid IL driver’s license required
  • Must be 16 years of age

*previous experience with behavior disorders and autism related issues preferred.
