SUBFUNCTION DEFINITION: Community Relations at Cincinnati Children's Hospital Medical Center serves as a point of entry into the organization as an information and resource hub that centralizes and standardizes the way we connect to the community.

PRIMARY JOB RESPONSIBILITIES
· Assist with review of transition resources and services.
· Work on development and implementation of a transition model from school to adult life with youth with autism, families/caregivers, and systems and services that impact transition success.
· Facilitate and maintain relationships with community partners and agencies for successful transition in the community.
· Plan and facilitate partner and advisory council meetings.
· Conduct community outreach and engagement.
· Assist with data collection and data entry, as well as report writing.
· Develop tools, materials, and resources; Disseminate information and resources


SECONDARY JOB RESPONSIBILITIES

·Program
Coordinate with leadership and internal departments to move strategic initiatives and meet established objectives around community engagement, community health and health promotion. Foster positive relationships among internal and external stakeholders and team in the areas to promote child health through community partnerships and employee engagement. May serve as the office-based contact person for the medical center's involvement with external community organizations and internal medical center units focused on quality improvement workforce development or pipeline programs. May assist with proposals and ongoing communications which address career exploration, employee development or systems transformation. Provides support & coordination including database development, data entry, reporting and fiscal management/tracking.
·Community Partnerships and Health Education
Knowledge of community engagement principles (best practices and effective strategies for collaboration with internal and community partners). May provide support for community education and health promotion. Demonstrated ability to work cooperatively and successfully with community organizations. Support strategic partnerships with community organizations, residents /patients and stakeholders that share a common mission with CCHMC. Support and attend CCHMC sponsored events as related to roles and goals.
·Community Engagement
Establish and maintain strategic partnerships with community organizations and stakeholders that share a common mission with CCHMC. May represent CCHMC on community boards and committees or organizations that support the strategic objectives of CCHMC. Support and attend CCHMC sponsored events as related to roles and goals.
·Training & communication
Prepare employee volunteers or community stakeholders through structured presentations and unstructured interactions for resolving questions. May facilitate employee knowledge of department and enterprise objectives by documenting regularly scheduled meetings and preparing appropriate reports. Effective verbal, written and interpersonal communications skills w/ the ability to provide and receive information from both internal and external audiences. Computer literate and working knowledge of Microsoft Office with emphasis on Powerpoint and Excel. Complete other training as required or needed to advance professional development skills.
Qualifications:

EDUCATION/EXPERIENCE


Required:
·Bachelor's degree in a related field
· 3-5 years experience working in a community engagement capacity with diverse communities
· Full-time availability (40 hrs/week), mostly daytime hours (Mon-Fri); some evenings and weekends; some local travel; reliable transportation is a must; hybrid schedule possible - some in person , on-site and community-based work required

· Reliable and energetic, takes initiative, enjoys working with youth, families, and professionals in/from the community.
· Interested/knowledgeable in/about Autism/developmental disabilities across the lifespan.
· Comfortable working in underserved areas with diverse communities.
· Desire to make a difference in the community as well as on a more systemic level.
· Excellent communication skills with youth, families, and professionals.
· Expertise in collaborative community-based approaches.
· Proficient computer/software skills (Microsoft Office 360).
· Proficient in using and managing virtual platforms (Zoom, Teams, etc.)
· Knowledge of communication/marketing and design tools a plus (e.g. Constant Contact, Canva, Photoshop, etc.).
· Being bi-lingual (e.g. Spanish, ASL) a plus.
· Full-time availability (40 hrs./week), mostly daytime hours (Mon-Fri); some evenings and weekends; some local travel. Reliable transportation a must. Hybrid schedule possible; some in-person, on-site and community-based work required.
