Job description


POSITION SUMMARY:

The Community Engagement Marketing & Partnerships (CEMP) Manager will lead FPCSN's community engagement, grassroots marketing, external partnership, and outreach strategies to strengthen the Network's presence within the communities it serves. Working collaboratively with health center leadership, staff, patients, community organizations, and external stakeholders, the CEMP Manager will develop and implement initiatives designed to increase awareness of FPCSN services, attract new patients, strengthen relationships with existing patients, reduce patient attrition, and connect community members with accessible, comprehensive healthcare.

The CEMP Manager will serve as a key representative of FPCSN throughout Philadelphia, cultivating relationships with community organizations, healthcare and social service providers, schools, elected officials, businesses, neighborhood associations, faith-based organizations, housing communities, and other strategic partners. The CEMP Manager will also support FPCSN's marketing and brand visibility efforts, ensuring that outreach activities translate into measurable community engagement, referrals, patient acquisition, retention, and organizational growth.

ESSENTIAL DUTIES & RESPONSIBILITIES:

Community Engagement & Patient Growth

  • Develop and execute a Network-wide community engagement strategy aligned with FPCSN's strategic priorities, service lines, patient access goals, and community needs.
  • Lead community-based initiatives designed to increase awareness of FPCSN services and generate new patient referrals and appointments.
  • Develop targeted outreach strategies for neighborhoods surrounding FPCSN health centers, school-based health centers, and priority service areas.
  • Identify opportunities to reconnect former or inactive patients with FPCSN services in collaboration with Operations, Outreach & Enrollment, clinical teams, and other appropriate departments.
  • Develop initiatives that strengthen patient loyalty, trust, and connection to FPCSN and support improved patient retention.
  • Identify barriers contributing to patient attrition, access challenges, or underutilization of services and elevate trends and recommended interventions to leadership.
  • Coordinate outreach efforts around new services, expanded appointment availability, patient portal adoption, self-scheduling, insurance enrollment, preventive health initiatives, and other Network priorities.
  • Ensure community engagement activities include clear pathways for community members to become FPCSN patients or reconnect with care.

 

Marketing & Brand Visibility

  • Support the development and implementation of FPCSN's community-facing marketing strategy in collaboration with the Chief Development & Strategy Officer and appropriate internal and external partners.
  • Develop grassroots marketing campaigns that increase awareness of FPCSN locations, providers, programs, services, and patient access options.
  • Coordinate distribution of promotional materials, rack cards, signage, flyers, digital content, and other marketing collateral throughout FPCSN service areas.
  • Identify opportunities for targeted promotion through community organizations, schools, businesses, housing communities, neighborhood associations, faith-based organizations, events, and other trusted community channels.
  • Support social media and digital marketing efforts by identifying community stories, events, partnerships, patient education opportunities, and other content that demonstrates FPCSN's impact.
  • Ensure community-facing materials and outreach activities consistently reflect FPCSN's approved brand standards, messaging, mission, and values.
  • Identify opportunities to increase FPCSN visibility through community events, sponsorships, speaking opportunities, health fairs, neighborhood initiatives, and strategic collaborations.
  • Maintain a community marketing calendar aligned with organizational campaigns, health observances, service promotions, and patient acquisition priorities.
  • Work collaboratively with Network leadership to develop targeted campaigns for services experiencing underutilization or opportunities for growth.

 

External Partnerships & Business Development

  • Proactively identify, cultivate, and maintain strategic partnerships that expand FPCSN's reach, visibility, referral network, and community impact.
  • Develop relationships with healthcare organizations, social service agencies, schools, universities, employers, housing organizations, community development corporations, businesses, faith-based organizations, civic associations, elected officials, and other community stakeholders.
  • Establish mutually beneficial referral relationships that create pathways for individuals and families to access FPCSN services.
  • Identify opportunities for organizational partnerships, community collaborations, sponsorships, co-branded initiatives, and other opportunities supporting FPCSN's strategic growth.
  • Maintain an organized database of community partners, referral sources, contacts, partnership activities, and potential opportunities.
  • Regularly assess the effectiveness of partnerships and prioritize relationships demonstrating meaningful community reach, referrals, resources, or strategic value.
  • Represent FPCSN at external meetings, coalitions, community forums, civic meetings, and partnership discussions.
  • Collaborate with Development and Strategy leadership to identify community partnerships that may support grant opportunities, philanthropic initiatives, or other organizational priorities.

 

Patient Acquisition, Retention & Referral Development

  • Establish measurable goals for outreach-generated referrals, new patient leads, appointments, community contacts, and other patient growth indicators.
  • Develop systems to track the progression from community outreach to referral, appointment scheduling, and patient establishment whenever feasible.
  • Monitor referral sources to identify organizations, events, neighborhoods, campaigns, and partnerships generating the strongest patient engagement.
  • Collaborate with Operations and clinical leadership to ensure marketing and outreach efforts align with available clinical capacity and appointment availability.
  • Support targeted patient recall and re-engagement initiatives in collaboration with appropriate departments.
  • Develop strategies to promote FPCSN services across the continuum of care so that existing patients are aware of additional services available throughout the Network.
  • Regularly analyze community feedback, patient engagement trends, and outreach results to recommend strategies for improving acquisition and retention.


Community Advisory Councils & Community Voice

  • Work with Center Directors to organize, strengthen, and facilitate Community Advisory Councils across FPCSN sites.
  • Recruit and cultivate Community Advisory Council members who reflect the populations and neighborhoods served by each health center.
  • Develop the FPCSN Ambassador Program utilizing Community Advisory Council members, patients, community residents, and volunteers to expand trusted community outreach.
  • Establish mechanisms for translating Community Advisory Council feedback into recommendations for service delivery, patient experience, communications, and organizational planning.
  • Ensure community feedback and emerging neighborhood needs are regularly communicated to appropriate Network leadership.


Community Events & Outreach Operations

  • Develop an annual Network-wide community engagement and outreach calendar.
  • Lead or coordinate FPCSN participation in community health fairs, open houses, neighborhood events, health education activities, enrollment initiatives, and other outreach opportunities.
  • Evaluate potential events based upon anticipated community reach, strategic relevance, cost, staff capacity, and potential patient acquisition.
  • Coordinate staffing, materials, messaging, promotional items, registration resources, and follow-up processes for community events.
  • Develop standardized processes for collecting contact information and facilitating follow-up with individuals expressing interest in FPCSN services.
  • Supervise and coordinate Community Engagement Specialists and other staff or volunteers supporting outreach initiatives


Data, Performance & Continuous Improvement

  • Maintain accurate tracking of outreach activities, community contacts, partnerships, referrals, appointments generated, and other established performance indicators in alignment with FPCSN's CQI Plan.
  • Develop monthly dashboards or reports summarizing community engagement, marketing, partnership development, referral activity, and patient growth outcomes.
  • Track return on investment and effectiveness of major outreach and marketing activities whenever feasible.
  • Identify trends and recommend adjustments when outreach strategies are not generating desired engagement or patient conversion.
  • Establish annual and quarterly community engagement goals aligned with FPCSN's Strategic Plan and organizational performance priorities.
  • Present community engagement outcomes, emerging needs, partnership opportunities, and recommendations to leadership as requested.


Supporting the Network

  • Maintain comprehensive knowledge of FPCSN locations, services, providers, programs, eligibility requirements, scheduling processes, patient portal functionality, and community resources.
  • Collaborate closely with Operations, Outreach & Enrollment, Quality, Clinical Services, Social Services, Development & Strategy, and health center leadership to advance patient access and community engagement priorities.
  • Support organizational initiatives requiring community outreach, patient communication, marketing, partnership development, or stakeholder engagement.
  • Serve as an FPCSN brand ambassador and ensure consistent, professional representation of the organization in all external interactions.
  • Perform other responsibilities related to community engagement, marketing, business development, partnerships, and organizational growth as assigned.


EXPERIENCE AND REQUIREMENTS:
Candidates should possess a diverse range of skills and experience including but not limited to:

Required Education and Experience

  • Required bachelor’s degree in social or human sciences, public policy or equivalent combination of training and experience related to the duties of this position. 
  • Preferred Master’s public health, health care administration, social work
  • 4 Years of Experience in Community Engagement 
  • 2 Years of Electronic Medical Records


Required Skills and Abilities

  • Success in engagement and collaboration at the individual, community and organizational levels, and strong interpersonal and relationship building skills, with demonstrated ability to engage others.
  • Experience with cross-sectional program design and systems thinking.
  • Extremely self-motivated, proactive attitude a must.
