Eden Senior Care, founded in 2016, is a growing Healthcare Management company, focused on managing and operating Skilled Nursing, Rehabilitation and Assisted Living communities in Minnesota, Wisconsin, Illinois, Ohio, Pennsylvania, Indiana and Alabama. Position: Hospital/Community Liaison

Edenbrook Wisconsin Rapids, a skilled nursing facility in the Eden Senior Care Network, is now hiring a Hospital/Community Liaison responsible for overseeing outreach and referral development for Friendly Village and Edenbrook Wisconsin Rapids, serving both the Wisconsin Rapids and Rhinelander communities. This role focuses on building strong relationships with hospitals and community partners, facilitating patient referrals, and supporting census growth through strategic outreach and admission coordination.

Key Responsibilities:

Hospital Liaison Responsibilities:
  • Complete daily rounds in designated hospitals
  • Obtain new high-acuity inquiries and assess patients to evaluate care needs prior to admission; direct referrals to the appropriate center
  • Evaluate inquiries, make admission-related decisions, and communicate information to the case manager and admissions coordinator to facilitate smooth patient transitions
  • Obtain accurate information from physicians, patients, and referral sources regarding expected discharge plans and communicate updates to the interdisciplinary team
  • Develop and maintain relationships with hospital social workers, physicians, discharge planners, administrators, clergy, attorneys, civic/professional organizations, home health agencies, and families to understand service needs and promote available programs
Business Development & Community Outreach:
  • Develop and maintain strong relationships with community organizations
  • Conduct routine in-person visits to assisted living communities, senior centers, senior high-rise apartments, and physician offices
  • Represent the facility at networking events and community programs
  • Provide education on facility services, specialty programs, and outcomes
  • Maintain a consistent presence in assigned territories
Census Growth & Strategy:
  • Monitor daily census and proactively identify opportunities for growth
  • Analyze referral and discharge trends and adjust outreach strategies accordingly
  • Participate in marketing planning and implementation
Qualifications:
  • Proven experience in healthcare admissions, marketing, business development, or case management (skilled nursing preferred)
  • Strong understanding of Medicare, Medicaid, and managed care reimbursement
  • Demonstrated ability to build and maintain relationships with physicians, hospital staff, social workers, discharge planners, and other referral sources
  • Sales and marketing experience with a track record of generating referrals and meeting performance goals
  • Excellent communication, negotiation, presentation, and relationship-building skills
  • Knowledge of local hospitals and healthcare networks preferred (willing to train)
  • Valid driver’s license and reliable transportation for local travel
