Job description

Community Connections Inc. is looking for a Registered Nurse to join our Day Center team based in Fall River, MA on a relief basis. In this role you will support individuals with disabilities by overseeing all health-care services provided to consumers, providing direct care and in-service training in relevant medical and health-related issues, and coordinating medical services with each consumer's primary physician or medical clinic. We are looking for a Registered Nurse with a commitment to care, integrity and serving the community.

What You'll Do as a Registered Nurse:

  • Dispense and document consumer medications
  • Participate in individual case record reviews on all consumers' with program's team
  • Conduct medication reviews regularly
  • Review all pertinent consumer health concerns weekly with Clinical Team
  • Monitor and record vital signs and weights every six months or as needed
  • Contact physicians, family/community residences, guardians, health care professionals and service coordinators as necessary regarding specific consumer medical issues
  • All other job-related duties as assigned

What You'll Need as a Registered Nurse:

  • Fully licensed with the Massachusetts Nursing Board
  • Bachelor's Degree from a four-year college or university preferred
  • 1+ year's experience in a community health setting
  • 1+ year's experience in a training program for individuals with developmental disabilities
  • Valid driver's license, access to a motor vehicle, and satisfactory driving record
  • Excellent verbal communication and interpersonal skills
  • Demonstrated ability to work independently and as a part of a team
  • Strong computer and word processing skills
