Position Overview

Community Health Connection is actively seeking a Licensed Clinical Social Worker (LCSW) to join our dedicated team in a full-time remote capacity. Work Environment

Key Responsibilities

• Conduct comprehensive psychosocial evaluations to assess the mental health needs of clients, documenting findings clearly and precisely.
• Provide individualized therapy sessions, adapting techniques such as cognitive-behavioral therapy or family systems therapy to suit each client's unique situation and cultural background.
• Engage in the formulation of detailed treatment plans that include measurable goals, direct interventions, and scheduled progress evaluations.
• Collaborate efficiently with a multidisciplinary team, including psychiatrists and nurses, to ensure cohesive care coordination and shared treatment objectives.
• Deliver crisis intervention services, executing immediate risk assessments and safety plans, particularly in high-stress situations.
• Advocate for clients’ rights and connect them with vital community resources, enhancing their access to additional support services.
• Engage actively in community outreach initiatives to elevate mental health awareness and combat stigma.

Patient Population / Client Focus

Your primary focus will be on supporting diverse populations within the hospice context, addressing both psychological and emotional challenges faced by patients and their families. This role empowers you to foster an environment of healing and support, helping clients navigate the complexities of life-limiting illnesses with compassion and understanding.

Qualifications

• Master's degree in Social Work from an accredited institution.
• Possession of an active license as a Licensed Clinical Social Worker (LCSW) in the state of Colorado.
• Demonstrated knowledge and understanding of cultural competence and sensitivity in providing care.
• Exceptional communication skills paired with a strong ability to empathize and collaborate with clients and team members alike.

Preferred Experience

• Prior experience in a community mental health or hospice setting is highly preferred.
• A proven track record of utilizing evidence-based practices in therapy, particularly in dealing with end-of-life emotional issues.
• Active engagement in professional development and a commitment to staying updated with best practices in social work and mental health.

Compensation and Benefits
