Position Description
As a Nurse Case Manager (RNCM), you will create, plan, and direct hospice care by utilizing the nursing process of assessment, intervention, implementation, and evaluation. You will collaborate closely with all members of the Interdisciplinary Team while building meaningful relationships with patients and their families. This role requires a commitment to the highest standards of care and professional nursing clinical competency.
Minimum Requirements
  • Current RN license in the State of Georgia
  • 1-2 years of nursing experience (preferred)
  • 1-2 years of hospice experience preferred
    (Candidates with experience in skilled nursing, home health, acute care, med-surg, or ICU will be considered)
  • Utmost compassion and integrity
  • Valid driver's license, insurance, and reliable vehicle
  • Excellent time management skills
