Housing Case Manager

Status: full time, permanent

Location:In office Newmarket and work in the York Region community.

Salary:$62,000-65,000 with benefits and pension


Purpose

WCYR is seeking a Housing Case Manager who understands the impact of the broader social determinants of health on people's ability to secure and retain housing and attend to their health and well-being. The Housing Case Manager will provide individual supports and short-term case management to community members who are homeless or at risk of losing their housing. The position requires the ability to facilitate community engagement and outreach, and create housing summits alongside community members, to better understand local needs, strengths and opportunities. The incumbent will work in Newmarket and across York Region


Duties and Responsibilities

  • Outreach and engage residents who are homeless or at risk of homelessness
  • Support community members to access health and social services through a warm transfer process
  • Develop partnerships with service agencies, ensuring efficient and timely access to services
  • Provide short-term case management, and make referrals to support community members with housing retention
  • Compile reports on housing needs and community recommendations
  • Develop and maintain directory of available housing, supports, and services
  • Explore and help develop a roommate matching program for community members


Requirements

  • Undergraduate degree, preferrably BSW/MSW
  • Minimum 1 year case management/short term case management experience
  • Minimum 1 year related experience supporting people with housing needs
  • Knowledge and appreciation of housing issues in York Region
  • Knowledge of legislation and regulations including Residential Tenancies Act
  • Knowledge of eviction prevention strategies
  • Knowledge of community resources and demonstrated ability to search for applicable resources/information
  • Must be able to work in office and to commute across York Region
  • Strong interpersonal skills, with an ability to work closely with clients and partner organizations
  • Problem solving skills in identifying and referring individuals to resources.
